Agency / Medical Provider / Escrow / Mental Health Provider / Other

Membership Requirements:

1.  Professional Liability insurance with 1M minimum coverage.

2. Active website that includes the names and photos of the Principal Owners and contact info. 

3. Confirmation that you’ve read and agree to abide by the SEEDS Standards. 

4. Company (no individual memberships) actively registered to do business in the USA. We are developing an international membership coming soon.

Your membership level should reflect the number of employees in your organization, not the number of people that will interact with SEEDS.

Note: Since SEEDS is a tax-exempt organization, we do not typically provide W-9 forms for membership dues, as these fees are considered a donation or membership support rather than a taxable service.
